Our state manager Penny Chugg is managing a project that seeks to review and evaluate current practices in case note formulation as well provide a quality framework which includes reporting mechanisms to monitor quality and consistency in case note writing.

Writing case notes is a task for which therapists often receive little or no training. Whilst the importance of documentation is clear, there is very little literature to inform therapists of what constitutes quality record keeping.
